Laboratory

We offer routine laboratory draws and testing for beneficiaries. Services are provided on a walk-in basis for patients with electronic orders placed in the system by Hurlburt and/or Eglin providers only. Referral orders from off base providers (civilian network) are scheduled by appointment only. Off base providers may submit signed orders via fax to 850-881-1792 or patients may drop a signed hard copy order at the clinic entrance. Patients will receive a call back for an appointment by laboratory staff within two business days. All patients must have a valid test order from an authorized provider prior to collection.
